After you accept admission to Reed and submit the first deposit, Reed will send you an email with information and instructions for setting up your computer account and email.
After you set up your computer account, you can access required forms online.
Below is a list of forms and action items that we ask you to complete in order to start your academic journey. Should you encounter issues completing the forms, please contact the Orientation team.

June 1
- Academic Advising Form*
This form will open on May 2, 2026. Identify your academic interests to help Reed match you with your first-year faculty adviser.
- Disability Accommodation Request Form (Housing)
This form is open on April 15, 2026. Complete this form to request accommodations for housing. Disability and Accessibility Resources (DAR) can accept housing accommodations requests from April 15 to June 1; however, after that date ability to implement accommodations is dependent on the available housing inventory.
- Disability Accommodation Request Form (Placement exams)
June 1 is the preferred deadline for this accommodation form. DAR will continue to process requests received after this date, as long as your accommodation request is submitted two weeks prior to the date when you will take your placement exam.
- Health Record and Vaccine Proof*
Review Reed's immunization requirements and health record requirements. Then, access the Student Health Portal to complete the required health forms and upload immunization history and/or exemptions (Reed requires proof of completed Measles, Mumps, and Rubella (MMR) series or exemption for the Oregon Health Authority).
- Housing: Residence Life Forms*
This form will be open on April 15, 2026, and is required for all incoming first-year students and transfers covered under our residency requirements. For more information about living on campus, visit the Residence Life website.
- Orientation Programs and Experiences: Peer Mentor Program
Reed's Peer Mentor Program (PMP) is designed to support students' transition to college by providing a pre-Orientation experience and introducing multicultural students in their first year at Reed to valuable campus resources and relationships. Apply by June 1 if you are interested in being matched with a peer mentor for your first year at Reed. The application opens on May 1.
- Photograph Submission for Reed ID*
Upload your photo for your Reed ID card. This is required for move-in.
- Second Enrollment Deposit*
Your first enrollment deposit was submitted when you accepted Reed’s offer of admission.
- Student Profile and Contact Information*
In IRIS, update your personal information (including your preferred first name, gender, pronouns, and orientation t-shirt size) and contact information (including your mailing address, phone number) and emergency contacts.
July 1
- Billing Information Release Form*
Authorize Reed to release detailed information about your college billing account to your parents, other family members, trustees, or banks and grant them online access to view and manage your student account.
- Campus Culture & Prevention Trainings*
In mid-July, you will receive an email with instructions. Completion required for all new students by August 24, 2026.
- Disability Accommodation Request Form (Academics)
This is the preferred deadline to request academic accommodations for fall semester courses.
- Disability Accommodation Request Form (Orientation)
If you have life-threatening food allergies or require accommodations to participate fully in Orientation activities, please fill out this form as soon as possible. We require three weeks’ notice, if possible, to ensure accommodations are available.
- Final High School Transcript Due to Admission*
Your high school email your final transcript to the admission office or you can mail it.
- Health Insurance Waiver Form
This form is required if you have comparable health insurance coverage and want to opt out of the Reed plan. If you do not submit a waiver, you will be charged for Reed's health insurance. The waiver portal is only available while in the U.S., and waivers typically take 3-5 business days to process.
- Move-In Arrival Time Sign-Up*
This form will be available on May 28, 2026. Students should sign up for a move-In time slot by July 1, 2026.
- Orientation Programs and Experiences: International Student Orientation Registration
Required for all F-1 and J-1 visa holders. Registration opens on May 28, 2026.
- Orientation Programs and Experiences: Outdoor Odyssey Experiences Registration
Registration opens on May 28, 2026.
- Orientation Programs and Experiences: Medical Information Form
This form will open on May 28, 2026, and is required for all Pre-Orientation and Outdoor Odyssey Orientation programs and experiences.
- Orientation Programs and Experiences: Waiver
This form will open on May 28, 2026, and is required for all Pre-Orientation and Outdoor Odyssey Orientation programs and experiences.
- Second Enrollment Deposit for Transfer Students*
Your first enrollment deposit was submitted when you accepted Reed’s offer of admission.
- Social Security or Tax ID Number*
Federal regulations require that students provide a Social Security number or Tax ID so the college can issue a 1098-T tax form. See IRS Publication 970 for more information. If you do not have a Social Security number or Tax ID, or do not wish to provide it, indicate that on this form.
- Tuition Billing
Check your Reed email for your tuition statement.
- Tuition Insurance Refund Plan Waiver
This form is required if you want to opt out of tuition insurance, which covers 70 percent of the term’s tuition and fees if you withdraw for medical reasons. Learn more about tuition insurance on our website.
