Hello, Reed staff and faculty.

I am writing today to ask you to report your COVID-19 vaccination status to the college. We request this for two reasons. First, college leaders need to know the percentage of staff and faculty who are vaccinated so we can make important decisions regarding campus procedures in the fall. Second, by law, we cannot adjust mask requirements for those who are vaccinated unless we first collect their vaccination data.

At your earliest convenience, take a photo of your vaccine document (the side with the vaccine dates) and then navigate to our vaccination verification tool. This works best on your phone.

• The tool operates in a browser. If you authenticate in Duo, you may need to close Duo and return to your browser window.
• Report the type, date, and state in which you received your vaccine
• Attach a photo of your vaccine document and then click “Close”
• Click “Submit” at the bottom.
