According to our records, you have not yet uploaded a complete copy of your COVID-19 vaccination record or valid exception to Reed’s health and counseling center (HCC) Student Health Portal. The August 1, 2021 deadline is fast approaching. If you believe you have received this message in error, please let us know.
As a reminder, all students are required to be fully vaccinated for the 2021–22 academic year with a World Health Organization*–authorized COVID-19 vaccine or have record of a valid exemption.

Not sure how to upload your record? Instructions for uploading immunizations to the Student Health Portal can be found here. Note that all documents must be translated into English before submitting. For assistance in uploading or faxing immunization documentation, contact the health and counseling center.
Not sure if the college already has your vaccination record? Students can view their current COVID-19 vaccine status on the Student Health Portal under the COVID-19 tab. Reed’s HCC may have automatically received notice of students who were vaccinated in the state of Oregon. If you have uploaded your COVID-19 vaccine card or were vaccinated in the state of Oregon and do not see the correct COVID-19 vaccine status on the Student Health Portal, contact the HCC via phone, email, or secure message.

* Students who do not have access to a WHO-approved vaccine, have already received a vaccine not approved by the WHO, or who have other extenuating vaccine circumstances should reach out to the HCC by email or secure message through the Student Health Portal to discuss and develop an individualized plan.
** The college will follow Oregon law in permitting students to request exemption from this vaccination requirement for medical or non-medical reasons. Contact the HCC by email or secure message for more information on applying for immunization exemptions.
