Indicate whether your parents or guardians may access your grades independently or will need to ask you for grade information.
- Billing Information Release Form*
Authorize Reed to release detailed information about your college billing account to your parents, other family members, trustees, or banks.
- Dewar Tuition Insurance Waiver
This form is required if you want to opt out of tuition insurance, which covers 70 percent of the term’s tuition and fees if you withdraw for medical reasons. Learn more about tuition insurance on our website.
- Health Insurance Waiver
This form is required if you have comparable health insurance coverage and want to opt out of the Reed plan. If you do not submit a waiver, you will be charged for Reed's health insurance. If you are under 18, you must complete a paper version of this form and obtain your parent or guardian’s signature.
- Health Record and Vaccine Proof*
Review Reed's immunization requirements and health record requirements. Then, complete required health forms through the Student Health Portal and upload proof of required vaccines or exemptions (Reed requires proof of COVID vaccine primary series/booster and MMR series).
- Housing Application*
Required for all first year and sophomore students. If you would like to request an exemption for on-campus housing, you should also use this link. Submit this form by noon on December 21.
- Photograph Submission for Reed ID*
Upload your photo for your Reed ID card. This is required for move-in.
- Social Security or Tax ID Number*
Federal regulations require that students provide a Social Security number or Tax ID so the college can issue a 1098-T tax form. See IRS Publication 970 for more information. If you do not have a Social Security number or Tax ID, or do not wish to provide it, indicate that on this form.
- Statement of Financial Responsibility*
Outlines the student’s personal responsibility for college charges assessed to their account.
- Student Profile and Contact Information*
In IRIS, update your personal information (including your preferred first name, gender, pronouns, and orientation t-shirt size) and contact information (including your mailing address, phone number) and emergency contacts.

After you accept admission to Reed and submit the first deposit, Reed will send you an email with information and instructions for setting up your computer account and email.

- New Student Check-in*
Let us know when you will be arriving on campus.
- Final High School Transcript Due to Admission*
Your high school can email your final transcript to the admission office or you can mail it.
- Sexual Assault Prevention Training*
Modules are available on Moodle. Complete by January 6 if possible, or by January 16 at the latest.
- Tuition Due Date
Check your Reed email for your tuition statement and view details online in the Student Account Center.
